What Common Skin Problems Do People Have Today?

Interviewer: What common problems in your holistic dermatology practice that affect people’s health the most and where do you think these were coming from?

Dr. Dattner: It’s interesting, there are a lot of things I call the social disease. We are eating 140 pounds of sugar per person per year in this country. That wasn’t always. We’re exposed to a tremendous number of pesticides of hormone disrupter in the plastics, and chemicals in the things that we put on our face. We are exposed to maybe different ultraviolet light than I was as a boy because of the breaking down of the ozone. There are so many things that are societal. A lot of us watch television on a regular basis so we get less exercise. Our whole way of living has shifted and it’s very different than it was 50 or a100 years ago. And these things all have their cost, and so a lot of what I am doing is just working against those damages. And it’s a social thing, at some point I hope that we begin to look a little bit more carefully at some of these things and say, is there a better way to do these things? Can we make our population healthier, more vibrant, more able to enjoy life more, produce more.

Interviewer: Yeah for sure, I’m on board for that. I mean the introductions of cell phones and computers, we are going in one direction and we are trying to pull in the other direction, where the world is thinking wellness, and the world is thinking, let’s prevent everything. And we come out with all of these things, it’s such a crazy world.

Dr. Dattner: Well it’s true, and we have to constantly be looking to figure out how to do that better.

Interviewer:  Yeah, got to get a balance there.

Dr. Dattner: Exactly

Interviewer: Okay doctor, do you have any last comment before we wind up here?

Dr. Dattner: Well I just wanted to remind people about radiant skin from inside out, because even when I see somebody for an hour, it’s hard to tell them everything that I have in a 300 page book. I think this would be very helpful for anybody who is interested in skin problems or inflammation in general. And I hope people will begin to find ways to really lead a healthier lifestyle and be more vibrant. And I think a lot of aging is just accumulation of damage that can be reversed. So change your attitude and take your life in your hands and improve it.

Gray Hair and Stress

6a00e55255b4628834017eea94cb01970d-200wi“You’re giving me gray hair!” a cry that I’m sure I was not the only one who heard this from their mother when their difficult behavior caused her stress. True or not, it all sounded a little like a folk tale, until now. New studies from the laboratory of Dr. Robert Lefkowitz at Duke University show that chronically elevated levels of the stress hormone adrenaline damage the DNA in specific ways that could lead to a spectrum of conditions from gray hair to tumor formation.

The damage was shown to break down p53 protein, the protein that protects the genome against cancer by helping potential cancer cells either repair their cancerous nature, or self-destruct.

In this report in the August 21 issue of the journal Nature, they also showed more detail of the pathway by which adrenaline cause this damage. This included the role of a molecule called beta–arrestin-1 in the process of causing the damage.Two major conferences were held at the New York Academy of Science in the late 1960’s on the effects of psychological factors in causing cancer. An impression of this effect was present for a long time, and supported by numerous studies.

This recent paper by Lefkowitz brings a new level of proof and understanding to the relationship between chronic, excessive stress and the onset of gray hair and cancer. While other factors certainly contribute to onset of these conditions, we can now pinpoint one factor over which we may have some control.
Exercise, meditation, breathing exercises and other stress-management techniques may contribute to longer, healthier, and less gray lives.

Natural Anti-Cancer Treatment: Eat This Food!

6a00e55255b462883401a3fcd5dbc9970b-200wiIf you’re looking for a way to balance out some of the negative effects of environmental toxins, a good way is to start eating more vegetables in the broccoli family.  This includes cabbage, cauliflower, collard greens, kale, arugula, and brussels sprouts among others.

Recent studies have demonstrated that broccoli’s chemical composition could help reduce the risk of cancer and, I believe, acne as well, because of the neutralizing effect these chemicals have on acne-triggering estrogen.

Broccoli is rich in vitamin C, carotenoids (vitamin A-like substances), fiber, calcium, and folate as well as phytochemicals that may have anti-cancer properties.
For example, broccoli contains several compounds called isothiocyanates, including sulforaphane and indole-3-carbinol (I3C), which are being explored for possible anti-cancer agents. Some studies have also suggested they
may alter body estrogen levels, which might affect breast cancer risk. Some studies have shown these substances may act as antioxidants and may boost the body’s detoxifying enzymes.

Another chemical in broccoli called indole-3-carbinol (I3C), seems to alter estrogen levels and may also raise levels of protective enzymes in the body. Lab studies have shown it may slow or stop the growth of breast, prostate, and other cancer cell lines.

Some early studies in animals have shown similar results. Small studies in humans have found it may prevent the development of precancerous growths in the cervix, as well as growths (papillomas) in the throat. While more, larger studies need to be done to confirm and expand our knowledge of the benefits, I recommend that people eat more broccoli and foods in the brassicaceae family for good health.

It is the beginning of the New Year, so I want to wish the best to all of you and to our world.

I would like to share some very exciting news with you. I was invited to go to India to be a featured speaker for an International Symposium on Integrative and Holistic Dermatology. To my knowledge, this was the first Symposium on Holistic and Integrative Dermatology sponsored by an academic Dermatology department, ever on the planet. On December 4, I gave three lectures on various aspects of holistic dermatology, and participated in a panel wrapping up the session at the Dermatology Department of Kasturba Medical College in Manipal University in Manipal, located in South West India. It was quite an experience to be honored by having the opportunity to teach about the approach to treating skin disease, and to do so to eager listeners halfway around the world. I have already heard from Dr. Shenoi, the head of the department, that what I presented was well received, and has given rise to new ways to treat difficult cases at the hospital there. In addition, my listeners were awakened to the neglected aspect of nutrition in Dermatology.

My knowledge was also enhanced by hearing the two other speakers, one of whom I had met when she was just entering this field of study.
I had a chance in Manipal to see the Ayurvedic department and the Yoga department at the medical center, which represents an integration of traditional care with conventional medical care that is rare in India as it is throughout the world. We spent a morning at a naturopathic hospital, which focused on diet and physical modalities for healing. Cleansing diet, simplicity, spirituality, and beauty of the surroundings, and yoga, were mainstays of the healing program there beyond the diet and other cleansing processes.

One of the other Integrative Dermatology speakers, Dr. S. I. Narahari, was having anopening of his new clinic for Applied Dermatology, and invited me to participate in the dedication and speak at his 5th annual National symposium. The physicians from all over India, honored mentor dermatologist Dr. Terrence Ryan from England,the press, and many people from the local government were present. More than 15 patients with elephantiasis were presented to a group of over 25 dermatologists and other physicians. Between presentations, I was asked by Dr. Narahari and a government minister to help design studies on nutrition and skin disease as, it relates to rural India. I am excited about being able to help those who might otherwise not receive care, and about extending studies in nutritional dermatology that can have a more widespread benefit.

To recover from jet lag on arriving in India, I spent the first 4 days in an Ayurvedic Spa, where I experienced the famous medicinal oil massages and hot herbal applications. We were treated to a visit to the associated third-generation manufacturing plant, which used age-old processes to manufacture Ayurvedic remedies. It has taken time for me to recover from the travels during flu season, so I finally writing to you now.

What has been most amazing to me are the similarities between several thousand-year old Ayurvedic concepts and what I am now doing, incorporating what is now being called functional medicine into my practice of dermatology. You Light Up My Life: The Mystery of DNA

6a00e55255b462883401901b96d12e970b-200wiThere is a possibility that some of our internal processes are being controlled by light–emitted by the DNA in our cells and transmitted in our own “fiberoptic” channels. The processes controlled may include everything from our symmetrical development in the embryo stage to signals being sent down acupuncture meridians.

A Korean doctor, Kim Bonghan, made these discoveries during the 1950’s and 60’s, and studies during this decade have identified these channels within lymphatic vessels using the latest scientific staining and microvisualization methods.

These channels may in fact be a vast, subtle communication system in the body that explains our reaction to light and acupuncture treatment. So when we say, “I see your light,” we may be visualizing a pattern of flowing light energy from our DNA’s activity down these light-transmitting channels described by Bonghan.

Food Allergy and Eczema (and Pregnancy)

6a00e55255b4628834017eea94c580970d-200wiAccording to an article published online ahead of print Oct 26 in the Journal of Allergy and Clinical Immunology, there was a significantly increased incidence of both food sensitization and filaggrin mutations in children who went on to developeczema and asthma.

A filiggrin is a protein related to penetration of foreign materials through the skin that has been found to be defective in a proportion of patients with atopic dermatitis, commonly known as eczema. The risk of being allergic to foods was up to 5 times as great in association with eczema as compared with those who did not have eczema.

Here’s what I have to say about this: despite the naysayers over the years who have claimed that eczema is not related to diet, this study from Germany shows an association between food allergy and eczema and asthma. I want to emphasize that in my experience, it takes more than food sensitivity testing and elimination to control food allergy manifestations.

Working with digestion, gut flora, and gut health, helps to reduce food allergy, and has proven to be helpful in reducing the incidence and severity of atopic dermatitis in infants, children and adults, as well as the severity of other inflammatory skin diseases.

Especially sensible is in normalizing these factors before starting pregnancy, if this planning is possible, and there is a history of atopy (asthma, hay fever, eczema) in you or your family. More about this important issue in a later blog.

Preventing Melanoma: Good for Skin Lines and the Bottom Line?

6a00e55255b4628834017d432073dc970c-200wiPrevention of sunburn may make be wise for your nest egg, not just for your skin. Melanoma, one of the fastest increasing forms of potentially deadly cancer, is related to sun damage to the skin. Once it goes beyond the local stage and spreads, the chances of survival are in the order of 15%.

The good news is that various genetic subtypes of melanoma are being identified, and targeted by specific drugs. More good news is that a new “biologic” drug has been developed by Genetech named Zelboraf which blocks a specific mutated protein found in certain melanoma known as BRAF 600 V. This protein can be identified by a mutation test to see if Zelboraf will be effective in their melanoma.

The bad news is that this drug is slated to cost $9800/month. Another related drug for late-stage melanoma recently approve named Yervoy costs $120,000/treatment course. If you think it will be easy to get your insurance to cover such treatment, just try to get them to pay for an out of network visit or lab test.
Especially for those with pale skin, lots of moles, and/or a family history of melanoma, this cost really questions the wisdom of overworking oneself to save money to reward oneself with sunburn vacations and weekends getting in the sun. Your wisest path, and most affordable path, is one of reasonable sun protection with hats and clothing as preventative investment. And don’t forget to get any suspicious lesions checked.To your health,

Taking Stock of Your Medical History

6a00e55255b4628834017eea94d3ce970d-200wiDiscovery of what illness a person has, and what is the underlying reason that illness is going on, often requires a lot of information.  Most of that information is related to your history, that of your family, and that of those around you.

No one knows this information better than you, so it is helpful to keep a record of what has been going on with your health:
·      Serious or recurrent illnesses, surgeries and accidents, medications, vaccinations and other treatments, and their dates
·      Supplements, vitamins, herbs, other alternative treatments and the apparent positive and negative effects of these treatments on you.
·      Diet, or changes or slips in your diet and their dates are also important.
·      Don’t forget to get copies of important lab test results, and recent lab tests results, even if negative.From my perspective, the events preceding your illness may be the most important clue to the cause of your specific illness that followed the exposure.

It may be more useful to edit the report you bring to a doctor, rather than bringing the whole file.  Try to keep a full file for yourself, and bring copies to your doctor, so that you keep a full set of your information.  Remember, doctors may only keep records for a period of up to 7 years.

Until there is a universal medical record kept, or perhaps a chip with that information in our wallet or under our skin, you are the best keeper and provider of information which may help with your health.

Put together your own health file, and keep it where you or your family can find it.

Melanoma Immunotherapy

6a00e55255b4628834017eea94de34970d-200wiI recently read a report in March in the journal Drugs in Dermatology about a man with metastatic melanoma (spreading skin cancer,) who had a partial regression of internal melanoma spreading. It occurred after applying a cream to the skin that was prescribed for lesser skin cancers.  The cream is Imiquimod, and was designed to stimulate the immune system to react against pre-cancers, warts, and occasionally, as in this patient, for basal cell carcinomas (the most common kind of skin cancer)

It works by stimulating a primitive form of the immune system, called “Toll-Like Receptors”  (TLRs), which are a very primitive form division of the immune system that are present not only in other mammals, but also in other species including insects.  Imiquimod works by stimulating a TLR called TLR 7, which then signals other branches of the immune system to join the battle and attack and destroy the tumor.

Interestingly, this patient could not tolerate other treatment he had been given, so, on his own,  he used the Imiquimod prescribed to him for a lesser skin cancer on the superficial metastatic lesions on his tummy skin, and the internal melanoma in his liver went away completely, and other internal melanomas got smaller.

The point of this comment is not to suggest Imiqimod for melanoma treatment; it may only work in certain cases, but rather to show that the right kind of stimulation of the immune system can make a great difference in reducing the size of potentially deadly cancers like metastatic melanoma.

The literature contains numerous reports and case reports of melanoma getting smaller or disappearing with other immune stimulants including chemicals like dinitrochlorobenzene (DNCB) and diphencyprone, and microbial stimulants such as Bacillus of Calmette and Guerin (BCG).

Not everyone responds to any given stimulus, so the challenge is to identify what the most likely stimulus is to get a melanoma rejected, and what else is necessary to get someone’s immune system to be strong enough to fight it off.

These are questions that I have been wrestling with in laboratories from Sloane Kettering to the National Cancer Institute over the past 5 decades.
